Thank you for registering to be part of a DELTA Ministries International Short-Term Missions Team. We are excited to see how God will work through you as you prepare and minister to others.
Please read the directions below carefully and fill out your registration form completely.
A deposit of $100, which will be applied toward your support, must accompany this registration.
1. Type your full legal name. This must match your passport name exactly.
If you don’t already have a passport, use the name that is going to appear on your passport. If you have a valid passport but have changed your name since it was issued, use the name on your passport, not your new name.
Your passport needs to match your airline ticket exactly.
2. We actually WANT you to READ the fine print! You must electronically sign the Legal Waiver before you can be a part of a DELTA Team. If you are under 18 years of age, you must print the Individual Registration Form, sign it, and have a parent sign as well. Please note that contributions, if you choose to have DELTA process your support donations, are non-refundable.
3. Complete the General Health section fully and honestly. This information will be used for emergency medical purposes only. In the event of an emergency, we need to have your current medical conditions and history.
If you are under the age of 18 you, and your parents/legal guardian, will need to download and complete a Minors Medical & Travel Release Form. You can get this release form from your team leader. If you are traveling out of the country, DELTA requires the Travel Release be notarized
