TEAM LEADER REGISTRATION

A minimum of $100 deposit is required for a team to be approved.  If known, please list all of the team members and pay the $100 deposit per person.    

MINORS MEDICAL & TRAVEL RELEASE 

This form must be downloaded and filled out by the parent of any team member less than 18 years of age.  *Please note that the Travel Release must be notarized. This is a requirement for entry into many countries. We will return the original of this form to you before your travel date.
TEAM LEADERSHIP

Make careful choices when you select your leadership. These individuals will be used by God to be directly involved in the careful discipleship, encouragement, and training of other Christians. Spend time praying about your choices.

Leadership Qualifications:
  • Have a commitment to prayer and Bible study
  • Have a history of committed church involvement
  • Must enjoy hard work and fun!

PASSPORTS

If your team is traveling overseas, each team member will need to obtain a passport. The processing time for a first-time passport is 10 to 14 weeks. An application can be obtained at your local post office or courthouse.
